the interview for this discussion involves a 35-year-old male with a history of morbid obesity with disabilities in a rural setting. Communication and interview techniques for this patient would differ from other patients based on the patient’s gender, young age, rural setting, and comorbidities. Being a mid-30-year-old male in a rural setting myself would give me common ground to relate to the patient and build rapport. Targeting questions based on the patient’s economic status might involve whether his disabilities leave him unable to work. Questions involving his education could be targeted by the rural setting and either poor access to or lack of finishing school for work or service-related reasons. Healthcare access could be targeted by questions revolving around the rural setting, physical distance to healthcare, and how the patient’s obesity affects transportation and healthcare access. Targeting questions on neighborhood and built environment could involve physical distance to food, exercise, or assistance and lack of developed areas for healthy activities. Social and community context questions would involve the patient’s beliefs and cultural attitudes towards healthcare, disability, and obesity, as well as food and lifestyle habits.

Health risks for this patient are many since morbid obesity is a risk factor for many illnesses and diseases. Major health risks include diabetes mellites (Type II), sleep apnea, depression, and many cardiovascular diseases. Chronic illnesses may be less pertinent to the patient’s age; preventing their development will be especially important. The CDC (2022) states that rural Americans are more likely to die prematurely from heart disease, cancer, unintentional injury, chronic lower respiratory disease, and stroke than their urban counterparts. What specific disabilities the patient suffers from will be important in determining how the other factors affect the patient’s risks.

A risk assessment instrument for identifying cardiovascular disease would be most appropriate for this patient. Early identification of cardiovascular disease could help prevent heart disease and stroke. Questions regarding this would revolve around symptoms such as tiredness, inability to perform tasks, shortness of breath, swelling, and more. Questions about physical activity, habits such as smoking, and diet would be important. Tests that would benefit in screening for cardiovascular risks would include blood pressure, cholesterol, and blood sugar (AHA, 2019).

Five targeted questions to ask this patient could be:

1. “What concerns do you have that brought you to seek medical care today?”

2. “Do you consider yourself to be overweight or unhealthy?” (This is a less open-ended question that directs the assessment toward the patients’ beliefs)

3. “Do you have any family members who have or have died from heart disease?” (Alternately: “Tell me about some of the health problems in your family.”)

4. “How often do you see a healthcare professional, and how difficult is it for you to seek medical care?”

5. “Are there any activities of daily living that are more difficult for you than they used to be?”

  • Assess the similarities and differences between Medicare and Medicaid managed care plans by comparing (a) strengths, weaknesses, and incentives; (b) commitment to access; and (c) risks to the consumers.
  • Synthesize two recommendations for improvement, including your justification, of each managed care plan. (You should have a total of four recommendations for both Medicare and Medicaid managed care plans.)

Must use at least three scholarly, peer-reviewed, and/or other credible sources published in the past five years in APA style.

As a case manager, what three principles will guide your work?

As a case manager, what three principles will guide your work? Provide a rationale for your choices.

In response to your peers, discuss how your choices are the same and how they differ.  See if you can come to an agreement on a final three including the rationale for choosing them.  Be sure to use proper APA format and include feedback that demonstrates critical thinking. Remember to introduce new ideas and perspectives and do not just agree or disagree with your peer’s thoughts.

Choose three of the five descriptions of history and discuss them from your own perspective and how you feel about the study of the past through the eyes of people.

-History does not repeat itself, but it often rhymes.

-History is a set of lies, agreed upon.

-History is the sum total of things that could have been avoided.

-History is a gallery of pictures in which there are few originals and many copies.

History repeats itself, first as a tragedy, then as a farce.

1. Choose three of the five descriptions of history and discuss them from your own perspective and how you feel about the study of the past through the eyes of people. Tie them together logically and discuss how you got to this point. How do you relate these things? Do you agree or disagree as a whole?

2. How would YOU describe history in the most succinct way possible. Use your own words, not text from online or outside sources. I am looking for your own point of view, creativity and insight into how you view things.

INITIAL POST should contain 300 words, substantive and addresses the prompt. Each of the the required secondary posts and commentary should EACH be at least 150 words of relevant and substantial responses

Identify and discuss the various factors that contribute to imperfect decision making in YOUR organization, the ARMY.

We all experience vulnerability at times with the decision making process. View the video,  The Power of Vulnerability.

Based on the video, how do we make ourselves vulnerable or not so vulnerable with decision-making. Sometimes do we try to make the uncertain, certain, by making a decision without the facts and taking into consideration everyone’s perspective of the decision at hand? Do we pretend that the other perspective doesn’t even exist when we are making decisions?

Identify and discuss the various factors that contribute to imperfect decision making in YOUR organization, the ARMY.

Summarize how the current account, capital account, and the official reserve account impact a country’s BOP.

Balance of Payments (BOP) is a method that countries use to manage international financial transactions. Different countries conduct this analysis at different time periods throughout the fiscal year. They use the BOP as a checks and balances system that assesses the current financial status of credits and debits of the country. Reflect on your reading from this week and consider how the current account, capital account, and the official reserve account components affect a country’s BOP.

With these thoughts in mind, address the following:

  • Summarize how the current account, capital account, and the official reserve account impact a country’s BOP.
  • Explain which condition you think is more desirable for a country—a BOP surplus or deficit. Support your response using this week’s Learning Resources.

What are the needs of your stakeholders that are relevant to your target population and need?

Develop a 4-6 page plan that will allow your intervention to be implemented in your target population and setting.

Introduction

Note: Each assessment in this course builds on the work you completed in the previous assessment. Therefore, you must complete the assessments in this course in the order in which they are presented.

Even the best intervention plan will not be effective without a sound and reasonable approach to implementing it. The implementation of the same intervention plan can vary drastically between different care settings, based on the culture of the care setting, the resources available, and the stakeholders involved in the project, as well as the specific policies already in place. A successful implementation plan blends contemporary and emerging best practices and technology with an understanding of the on-the-ground realities of a specific care setting and the target population for an intervention. By synthesizing these various considerations it is possible to increase the likely success of the implementation and continued sustainability of an intervention plan.

Preparations

  • Read Guiding Questions: Implementation Plan Design [DOC]. This document is designed to give you questions to consider and additional guidance to help you successfully complete this assessment.
  • As you prepare to complete this assessment, you may want to think about other related issues to deepen your understanding or broaden your viewpoint. You are encouraged to consider the questions below and discuss them with a fellow learner, a work associate, an interested friend, or a member of your professional community. Note that these questions are for your own development and exploration and do not need to be completed or submitted as part of your assessment.
    • What are the needs of your stakeholders that are relevant to your target population and need?
    • What applicable health care policy and regulations are relevant to your target population and need?
    • How will these considerations impact the development of your Intervention Plan Design assessment?
    • How can you work these considerations into the development of your Implementation Plan Design assessment?

Instructions

Note: The assessments in this course are sequenced in such a way as to help you build specific skills that you will use throughout your program. Complete the assessments in the order in which they are presented.

Your implementation plan design will be the third section of your final project submission. The goal for this is to design a plan that will allow your intervention to be theoretically implemented in your target population and setting. You should be able to preserve the quality improvement outcomes that you designed for your target population and setting while also ensuring that the intervention does not put undue stress on the health care setting’s resources or violate any policies or regulations. Provide enough detail so that the faculty member assessing your implementation plan design will be able to provide substantive feedback that you will be able to incorporate into the final draft of your project.

At minimum, be sure to address the bullet points below, as they correspond to the grading criteria. You may also want to read the scoring guide and the Guiding Questions: Implementation Plan Design document (linked above) to better understand how each criterion will be assessed. In addition to the bullet points below, provide a brief introduction that refreshes the reader’s memory about your problem statement, as well as the setting and context for which this intervention plan was designed before launching into your implementation plan.
