Translational Science Model and Project Management Plan
Identify the theory or model that will serve as the framework for the project; translational science models are recommended.
Identify the primary concepts or steps of the model.
Create the steps in your DNP practice change project that coincide with the theoretical concepts.
Include the timeframe for each step in your project plan (e.g., “in week 1 the data before intervention implementation will be collected”). Describe the project management plan by starting with the timeline of weeks for the project (usual projects are 8-12 weeks, keeping in mind the requirement to implement the intervention for at least eight weeks).
The implementation plan must include a full eight weeks of intervention implementation.
Include the plan for the pre-and post- summative data collection.
Include the weekly plan for the formative evaluation to ensure high intervention fidelity. Develop a week-to-week formative evaluation plan utilizing observation, 1:1 discussions, huddles, leadership meetings, or other processes to describe your project management oversight during implementation.
For each step or activity in the intervention plan, identify the intervention week in which that activity will occur.
Plans for Sustainability
Create your detailed plan to sustain the project over time.
Identify the specific strategies used to ensure the project has ongoing evaluation and modification as needed to ensure its success after your implementation phase is complete.
Anticipated Outcomes
Provide an in-depth discussion connecting the outcome(s) you will measure with determining the effect of the intervention on the outcome.  The articles that provide the evidence for the intervention can also provide the support of the outcomes. Use the same outcomes from the intervention research articles. Describe how researchers have tested the intervention on a specific outcome and how we know that the intervention is effective in achieving the targeted outcome.
If you plan to use a survey or questionnaire for a data collection tool, state the name of the data collection tool.
If you plan to use a survey or questionnaire for a data collection tool, state if permission is required to use that tool and provide the permission letter or email as an appendix. If permission is not required, state the tool is in the public domain and include the citation and reference for the tool.
Describe the survey/questionnaire tool identifying the number and type of questions, how it will be administered, how much time it will take for participants to complete and the reliability and validity data statistics of the tool, with a citation from the original authors who validated the tool.
If abstracting data from the Electronic Health Record (EHR), the Enterprise Data Warehouse, or a database, state the outcome data that will be abstracted and who will pull the data, when, and how.

What would you have done differently if you had foresight of your presentation?

Family Life Education Project
Part 3: Evaluation Report of Presentation ‐ 50 points
The purpose of this evaluation is to identify the learning experiences that came about in the topic selection process, the research process, preparation of the presentation, the actual presentation, and the responses/ feedback from the audience. After identifying these factors, the next step is to synthesize the information into suggestions for affirmation and improvement for future integration into family life education work.

This report will be divided into the following headings: Preparation Process
Presentation Experience Recommendations for Improvement

Under each heading, identify what you learned in the process of the project. Below are some sample questions that you may use to examine each stage in the process. These are just suggestions. You may have more significant comments on which to focus.

Preparation Process:
In selecting a topic, is there anything you would do differently in retrospect? Would you select a different topic or a different direction of the same topic? Would you seek out different types of academic literature?
If so, why and what would you look for the next time (expand on this in your “Recommendations” section)?

Presentation Experience:
Where would you present this information? (community setting of some type, school, etc.)
What size audience do you think is best suited for this presentation?
What do you think about your use of technology?
Was it easy or difficult? Did it add or distract? Would you use more or less technology in the future?

Recommendations for Improvement:
What would you have done differently if you had foresight of your presentation?
Name at least 2 areas where you could improve or strengthen an area of your project for future use. Do you have advice for a student starting out with this project?

Determine if any mitigation activities are required and explain why.

The top two threats have occurred.
The top opportunity has been realized.
The project’s risk budget is already exhausted.
The risk management schedule has been shortened by two months.

Write a 4–6 page paper in which you:

Analyze the impact of the events on the project.
Determine if any mitigation activities are required and explain why.
Determine if budget or schedule changes are necessary and explain why.
Update the risk register and highlight the changes made. Provide justification for the changes.
Use at least four quality resources in this assignment. Note: Wikipedia and similar websites do not qualify as quality resources.

Preamble to the Universal Declaration of Human Rights
(Source: Preamble to the United Nations Universal Declaration of Human Rights)
Whereas recognition of the inherent dignity and of the equal and inalienable rights of all members of the human family is the foundation of freedom, justice and peace in the world,
Whereas disregard and contempt for human rights have resulted in barbarous acts which have outraged the conscience of mankind, and the advent of a world in which human beings shall enjoy freedom of speech and belief and freedom from fear and want has been proclaimed as the highest aspiration of the common people,
Whereas it is essential, if man is not to be compelled to have recourse, as a last resort, to rebellion against tyranny and oppression, that human rights should be protected by the rule of law,
Whereas it is essential to promote the development of friendly relations between nations,
Whereas the peoples of the United Nations have in the Charter reaffirmed their faith in fundamental human rights, in the dignity and worth of the human person and in the equal rights of men and women and have determined to promote social progress and better standards of life in larger freedom,
Whereas Member States have pledged themselves to achieve, in cooperation with the United Nations, the promotion of universal respect for and observance of human rights and fundamental freedoms,
Whereas a common understanding of these rights and freedoms is of the greatest importance for the full realization of this pledge,
Now, therefore,
The General Assembly,
Proclaims this Universal Declaration of Human Rights as a common standard of achievement for all peoples and all nations, to the end that every individual and every organ of society, keeping this Declaration constantly in mind, shall strive by teaching and education to promote respect for these rights and freedoms and by progressive measures, national and international, to secure their universal and effective recognition and observance, both among the peoples of Member States themselves and among the peoples of territories under their jurisdiction.
