Describe the circumstances surrounding the difficult work-related decision you had to make.

Managers make numerous decisions on a daily basis. For some, the best choice is easy or straightforward to determine. If a machine breaks down, you either need to repair it or replace it. In an increasingly interconnected and complex world, however, the best decisions can often be unclear. Consider where you would stand on the following scenario:A company has developed a new product for which there is a great demand. The consensus of the leadership team is that the company should capitalize on this opportunity and hike up the price to generate a 300% return on each sale. They feel that, as long as the marketplace is willing to pay for it, then there is no problem, especially since this would help solve recent discussions of layoffs due to declining sales. Only one person in the leadership team voices disapproval of this approach, saying that this is a greedy action that would rip off customers and go against the company’s values. The person feels that—despite what it would mean for the business—the product should be priced to generate a 40% return, which is an average return for the company’s products.RESOURCESWEEKLY RESOURCESTo prepare for this Discussion:Identify a time you had to make a tough decision related to work. This should be a time when the right course of action was not immediately apparent or clear-cut. There may have been differing points of view, each with some validity, or perhaps there were some special circumstances in the situation that made the correct and ethical choice less obvious.Consider the values, guidelines, and rules that you used to make that decision.BY DAY 3Post a synthesis of a difficult work-related decision you had to make, including how you might approach the situation in the future. Specifically:Describe the circumstances surrounding the difficult work-related decision you had to make. In your description, include details about the options you were weighing, why the decision was difficult, et cetera.Explain the guidelines, rules, and values you used to make the decision.

One common dilemma faced by psychiatric nurse practitioners is determining who the “client” is. Given that children are typically brought for treatment by their parents, describe how the psychiatric nurse practitioner would respond to a child who discloses they are indulging in illicit behavior and asks you not to tell his or her parents.

James is a 60-year-old male with a known history of chronic obstructive pulmonary disease (COPD). He has a 30-pack-year history of smoking but quit 10 years ago. He has been compliant with his medications, which include an inhaled corticosteroid/long-acting beta-agonist (ICS/LABA) combination inhaler and short-acting beta-agonist (SABA) rescue inhaler. John lives alone and manages his activities of daily living independently. Presenting Symptoms: 1. Shortness of Breath: James reports increasing shortness of breath over the past three days. He states that even simple tasks like getting out of bed or walking to the bathroom leave him breathless. 2. Increased Sputum Production: He notes a significant increase in the amount of yellow-green sputum he’s been coughing up, which is thicker and stickier than usual. 3. Chest Tightness: James describes a sensation of chest tightness and increased use of his SABA inhaler, but it provides only temporary relief. 4. Decreased Exercise Tolerance: He mentions that he used to take short walks daily, but now he struggles to even leave his bedroom. Vital Signs: Temperature: 98.6°F (37°C) Heart Rate: 100 bpm Respiratory Rate: 28 breaths/minute Blood Pressure: 140/90 mm Hg Oxygen Saturation: 88% on room air Physical Examination Findings: 1. General Appearance: James appears anxious, sits leaning forward with pursed lips, and uses accessory muscles for breathing. 2. Respiratory Assessment: o Diffuse wheezing is heard upon auscultation. o Increased anterior-posterior chest diameter is observed. 3. Cough: John has a productive cough, and his sputum is yellow-green and purulent. Laboratory Findings: 1. Arterial Blood Gas (ABG): o pH: 7.32 o PaCO2: 60 mm Hg o PaO2: 56 mm Hg o HCO3-: 30 mEq/L o SaO2: 88% 2. Complete Blood Count (CBC): o White Blood Cell (WBC) Count: 14 o Platelets 150 3. Basic Metabolic Panel (BMP): o Sodium (Na): 139 o Potassium (K): 3.1 o Bicarbonate (HCO3): 30 mEq/L o Creatinine: 2.1 Chest X-ray: The chest X-ray reveals hyperinflated lungs with flattened diaphragms and increased retrosternal airspace, consistent with COPD. 1. What are the differential diagnoses for this patient? 2. 2. What are the known diagnoses for this patient? 3. What is the plan for this patient? 4. Compare and contrast the workup, diagnosis, and treatment for a patient with asthma vs a patient with COPD.

The number of people who are released from state and federal prisons each week in the United States is staggering. Research tells us a high percentage of those released from prison will re-offend and return to prison or jail. There has to be a solution to the prison’s revolving doors. The criminal justice policy on correctional treatment and rehabilitation may be the solution to this problem. What if those offenders preparing to exit prison completed a correctional treatment program that enabled them to be successful with their families, in their finances, and employment? What if those convicted on minor, non-violent crimes were able to receive rehabilitation that empowered them to become contributing members of society and never return to jail?
