Consultation in clinical practice

Consultation is crucial in clinical practice as often times as clinicians we can feel stuck in our intervention strategies with our clients and unintentionally begin to have tunnel vision as it relates to the overarching themes originally presented by our clients.  Consultation provides an opportunity for our colleagues, who vary in experience and intervention skills, to provide unbiased feedback and insight to help us reframe our intervention strategies and can also provide a space for clinicians to explore any emotional experience they are having in working with their client, that may or may not, be impacting our work with them.  This assignment is intended to help students practice how to formulate presenting a case to a consultation group and in turn, learning how to provide a brief, clinical analysis feedback to colleagues seeking consultation.

One of my strengths regarding professional presentations is good communication. Having good communication will allow me to engage with my audience, get them interested and give them a reason to listen. One way I can continue to build this strength is by becoming comfortable with sharing my personal experiences. My presentation is on increasing breastfeeding support through increasing nurse knowledge on the benefits of breastfeeding. I myself have two children, and had completely different breastfeeding experiences. With my second child, I was better educated and was able to seek support to which this allowed me to successfully exclusively breastfeed. Humans are programs to respond to stories. Telling stories is a compelling way of presenting because humans relate to them, they have the power to engage the audience, evoke empathy, increase trust and motivate action (Barnard, 2018).

One of my weaknesses regarding professional presentations would be that I must prepare well for a presentation for if I don’t then I will easily stumble on my own words. When I start to stumble, I start using words like “um” or use my hands too much and then anxiety kicks in. Fear of public speaking or public speaking anxiety is known as glossophobia. There are a number of strategies that can be used to cope with speech anxiety and become better at public speaking in general for example pre-performance planning, practice and visualization and setting goals (Cuncic, 2021). Rehearsing my presentation many times to adjust order and timing can give me the practice and confidence I need to avoid stumbling.

Identifying strengths and weakness will allow me to improve on my future presentations. When presenting in a formal setting you want to make sure your presentation is effective enough to get through to your audience and that they can use the information you are presenting. This is also a good leadership skill that can ultimately lead to personal success or the success of the organization you are working with.

Discuss one personal strength and one weakness you have regarding professional presentations. Name one method for improvement for each of these and discuss why it is important for you to work on these skills if you want to present your findings in a more formal setting.

Evidence-based practice (EBP)

Evidence-based practice (EBP) is an approach to healthcare that utilizes the most current research available in order to improve the health and safety of patients while reducing overall costs and variation in health outcomes (Black, et al., 2015). Introducing change, even evidence-based practice, is not always readily accepted by everyone and may require time for adjusting. Additionally, there may be barriers that may prevent successful implementation of evidence-based practice. Two main barriers to breastfeeding support provided by nurses to new mothers while in the hospital setting are organizational support and lack of motivation from staff members.

Support for the implementation of evidence-based practice is the primary barrier. Nurses need more than to just be given the information, shown how to implement and told to do it. The Knowledge, attitude towards implementing evidence-based practice, lack of resources and training, time mismanagement and lack of motivation are barriers to the implementation of evidence-based practice (Dagne & Beshah, 2021). Organizational support is vital; this can be done by assigning a lighter workload so that nurse can have sufficient time to help these mothers or an extra staff member. Having resource information at hand can also be useful so that nurse can provide this to mothers. Providing nurses with the tools they need to accomplish this practice will motivate nurses to partake in the practice.

Although breastfeeding is natural it may not come natural to all mothers. These mothers may come across challenges such as trouble latching, pain when latching, sore nipples, etc. The goal is to provide nurses with all the resources needed to help and provided these mothers with support they desperately need. But if the nurses are not provided support themselves, they will not be motivated to follow through and this change proposal will fail. Being short staffed, heavy workloads, lack of resources, insufficient time and unclear or unattainable high expectations can discourage nurses from implementing evidence-based practice.

Sustaining change can be difficult, as there are many variables that can affect implementation. One critical component of EBP is to ensure that practice change is part of an organization’s culture so it will continue to impact outcomes over time. Name two potential barriers that may prevent your EBP change proposal from continuing to obtain the same desired results 6 months to a year from now, and your strategies for overcoming these barriers.
