Please read this background information and answer the questions presented below. 
Andrea (Kennedy) Yates was born on July 2, 1964 in Houston, Tex. She graduated from Milby High School in Houston in 1982. She was the class valedictorian, captain of the swim team and an officer in the National Honor Society. She completed a two-year pre-nursing program at the University of Houston and then graduated in 1986 from the University of Texas School of Nursing in Houston. She worked as a registered nurse at the University of Texas M.D. Anderson Cancer Center from 1986 until 1994. 

Andrea Meets Rusty Yates: 

Andrea and Rusty Yates, both 25, met at their apartment complex in Houston. Andrea, who was usually reserved, initiated the conversation. Andrea had never dated anyone until she turned 23 and prior to meeting Rusty she was healing from a broken relationship. They eventually moved in together and spent much of their time involved in religious study and prayer. They were married on April 17, 1993. They shared with their guests that they planned on having as many children as nature provided. 

Andrea Called Herself “Fertile Myrtle”: 

In their eight years of marriage, the Yates had five children; four boys and one girl. Andrea stopped jogging and swimming when she became pregnant with her second child. Friends say that she became reclusive. The decision to home-school the children seemed to feed her isolation. 

The Yates Children: 

Feb. 26, 1994 – Noah Yates, Dec. 12, 1995 – John Yates, Sept. 13, 1997 – Paul Yates, Feb. 15, 1999 – Luke Yates, and on Nov. 30, 2000 – Mary Yates was the last child to be born. 

Their Living Conditions: 

Rusty accepted work in Florida in 1996 and the family moved into a 38-foot travel trailer in Seminole, FL While in Florida, Andrea got pregnant, but miscarried. In 1997 they returned to Houston and lived in their trailer because Rusty wanted to “live light.” The next year. Rusty decided to purchase a 350-square-foot, renovated bus which became their permanent home. Luke was born bringing the number of children to four. Living conditions were cramped and Andrea’s insanity began to surface. 

Michael Woroniecki: 

Michael Woroniecki was a traveling minister from whom Rusty purchased their bus and whose religious views had influenced both Rusty and Andrea. Rusty only agreed with some of Woroniecki’s ideas but Andrea embraced the extremist sermons. He preached, “the role of women is derived from the sin of Eve and that bad mothers who are going to hell create bad children who will go to hell.” Andrea was so totally captivated by Woroniecki that Rusty and Andrea’s family grew concerned. 

Andrea’s First Suicide Attempt : 

On June 16 1999, Andrea called Rusty and begged him to come home. He found her shaking involuntarily and chewing on her fingers. The next day, she was hospitalized after she tried to commit suicide by taking an overdose of pills. She was transferred to the Methodist Hospital psychiatric unit and diagnosed with a major depressive disorder. The medical staff described Andrea as evasive in discussing her problems. However, on June 24 she was prescribed an antidepressant and released. 

Spiraling Downward: 

Once home, Andrea did not take the medication and as a result she began to self mutilate and refused to feed her children because she felt they were eating too much. She thought there were video cameras in the ceilings and said that the characters on television were talking to her and the children. She told Rusty about the hallucinations, yet neither of them informed Andrea’s psychiatrist, Dr. Starbranch. On July 20, Andrea put a knife to her neck and begged her husband to let her die. 

Warned About the Risks of Having More Babies : 

Andrea was again hospitalized and stayed in a catatonic state for 10 days. After being treated with an injection of different drugs that included Haldol, an anti-psychotic drug, her condition immediately improved. Rusty was optimistic about the drug therapy because Andrea appeared more like the person he first met. Dr. Starbranch warned the Yates that having another baby might bring on more episodes of psychotic behavior. Andrea was placed on out-patient care and prescribed Haldol. 

New Hopes for the Future : 

Andrea’s family urged Rusty to buy a home instead of returning Andrea to the cramped space of the bus. He purchased a nice home in a peaceful neighborhood. Once in her new home, Andrea’s condition improved to the point that she returned to past activities like swimming, cooking and some socializing. She was also interacting well with her children. She expressed to Rusty that she had strong hopes for the future but still viewed her life on the bus as her failure. 

The Tragic End: 

In March of 2000, Andrea, on Rusty’s urging, became pregnant and stopped taking the Haldol. On November 30, 2000, Mary was born. Andrea was coping but on March 12, her father died and immediately her mental state digressed. She stopped talking, refused liquids, mutilated herself, and would not feed Mary. She also frantically read the Bible. 

By the end of March Andrea returned to a different hospital. Her psychiatrist, Dr. Mohammed Saeed, treated her briefly with Haldol but discontinued it, saying that she did not did not seem psychotic. Andrea was released only to return again in May. She was released in 10 days and in her last follow-up visit with Saeed, she was told to think positive thoughts and to see a psychologist. 

Two days later, Rusty left for work and before his mother arrived to help, Andrea began to put into action the thoughts that had consumed her for two years. 

Andrea filled the tub with water and beginning with Paul, she systematically drowned the three youngest boys, then placed them on her bed and covered them. Mary was left floating in the tub. The last child alive was the first born, seven-year-old Noah. He asked his mother what was wrong with Mary, then turned and ran away. Andrea caught up with him and as he screamed, she dragged him and forced him into the tub next to Mary’s floating body. He fought desperately, coming up for air twice, but Andrea held him down until he was dead. Leaving Noah in the tub, she brought Mary to the bed and laid her in the arms of her brothers. 

During Andrea’s confession she explained her actions by saying that she wasn’t a good mother and that the children were “not developing correctly” and she needed to be punished. 

Her controversial trial lasted three weeks. The jury found Andrea guilty of capital murder, but rather then recommending the death penalty, they voted for life in prison. At the age of 77, in the year 2041, Andrea will be eligible for parole.

The Occupational Safety and Health Administration’s (OSHA) Process Safety Management of Highly Hazardous Chemicals (PSM) standard (29 C.F.R. § 1910.119) contains requirements for the safe management of chemicals for facilities that exceed OSHA’s threshold quantities for highly hazardous substances. This standard requires the establishment of a PSM program that contains 14 elements for the safe management of hazards associated with the use, storage, manufacturing, handling, or moving of highly hazardous chemicals.
You have recently been promoted to the position of health, environmental, and safety (HES) manager at Marz Pulp and Paper, Inc. (MPP), a pulp and paper manufacturer that is located in the Pacific Northwest region of the United States. The company employs 120+ management, supervisory, operations, and ancillary personnel, including contractor employees from 15 different companies. MPP uses the kraft pulping process to convert wood chips to a pulp for use in their paper production process. The pulp mill is considered a covered facility pursuant to the PSM standard (29 C.F.R. §1910.119). You have been accompanying an OSHA inspector this past week on a PSM inspection of your pulp mill process facilities, including the review of all PSM-related operating procedures, process information, hazardous material, mechanical integrity inspection, and incident and training records. The OSHA inspector has completed their inspection of your facility and presented the following preliminary observations during the PSM inspection in the briefing for your facility management and operations supervisors:

Although it will be several weeks before OSHA provides the formal PSM inspection report and findings, you plan to proactively schedule an internal follow-up meeting with your management team and operations supervisors to prepare them for what to expect, including possible citations and fines for violations of the PSM standard. The owner of a privately held medical pharmaceutical company whose name is CVS and has a successful past coming up with medical solutions to current challenges regarding severe diseases. Recently they decided to develop a COVID-19 vaccine that will be 98% effective and can be ready in the next 90 days. Prior to the need for the vaccine, CVS has been using R&D to be prepared in case of a pandemic occurring in the world. The FDA knows the company well and feels they have the capability to make this happen based on their history.

There is a lot of excitement for the company since revenues have dropped this past year and their earnings have flattened out. The owner feels this is the breakthrough they have been waiting for. Some major financial decisions need to be made to make this happen and become a reality. As the CFO of the company, you are tasked to do an analysis of the financial situation and present to the board some recommended financial steps for the company to finance this new project.

John Angelo Gotti III is a New York City mobster who during the 1990s led the Gambino crime family of Cosa Nostra for his imprisoned father John J. Gotti. He is also known as the son of the Teflon Don.  John Gotti Jr. arrested on murder conspiracy charge. Barcelo for NewsJohn “Junior” Gotti escorted Tuesday morning outside of the Melville, NY, FBI building. John A. (Junior) Gotti was ratted out by a boyhood buddy who gave the feds enough evidence to charge the mob heir Tuesday with three murders – and possibly put him away for life, sources told the Daily News. 

John Alite, who got his start in crime robbing drug dealers with the Dapper Don’s son, was not named as a defendant in the indictment charging Junior with racketeering, murder and cocaine trafficking. Alite, 45, however, was identified by the feds as “a powerful associate of the Gambino crime family” who directed their “criminal enterprise” in Tampa. A career criminal from Ozone Park, Queens, who fled the country in 2004 after he was hit with federal racketeering charges, Alite vowed in an interview two years ago he’d never rat on Gotti. But he also said he feared going to prison. 

“Either way, I’m a dead man,” Alite told the St. Petersburg Times as he sat in a stinking Brazilian jail and fought extradition to Florida for two years. Since then, Alite, who was extradited in December 2006 and could face a murder rap, appears to have decided that crossing Gotti is less dangerous than going to jail, the sources said. 

Gotti, 44, was arrested by the FBI at his Long Island home at 6 a.m. Tuesday. Though the murders happened in New York, the charges grew out of a decade-long investigation of the Gambino family’s operations in Tampa. Displaying none of his late father’s sartorial splendor, Junior wore a navy-blue polo shirt and matching sweatpants at his arraignment. “Yes, Your Honor,” he politely answered the judge. Manhattan federal prosecutor Elie Honig said at a hearing that the indictment against Gotti “captures a lifetime of criminality,” tracing his rise from a street-level enforcer in the 1980s to a cocaine king with a “stable” of dealers in Queens. 

Honig said Gotti became a “made man” after ordering the December 1988 murder of George Grosso in Queens and “oversaw” the murder of Gambino family soldier Louis DiBono, who was rubbed out on Oct. 4, 1990, in a parking garage of the World Trade Center. John Gotti, the Gambino family boss who died behind bars in 2002, was caught on tape by the feds approving the demise of DiBono because “he refused to come in when I called.” 

Junior was also charged in the Nov. 20, 1991, slaying of Bruce Gotterup in Rockaway Beach, Queens. “Mr. Gotti absolutely denies any involvement with any of the three murders,” Gotti attorney Charles Carnesi said. “There is no evidence. … This is a case based on a drug dealer who wants to avoid going to jail.” 

Carnesi said the feds were trying to pin the murders on Gotti because they tried and failed three times to convict him of plotting to kidnap Guardian Angels founder Curtis Sliwa. He said Gotti was a family man who has stayed out of trouble since his 1999 conviction on racketeering charges. 

The judge was not swayed by Carnesi’s words. He ordered Gotti held without bail and extradited to Tampa for trial. Carnesi said the feds have not set a date to send Gotti to Florida and that he’s going to try to have him “plead not guilty by video … so he can stay up here, at least.” Gotti’s mother, Victoria, told The News she was “just wiped out.” His sister Victoria questioned why Gambino turncoats Salvatore (Sammy Bull) Gravano and Michael (Mikey Scars) DiLeonardo didn’t tell the feds “anything about those crimes.” “The feds first tried to make John, my brother, a rat – yet he stood trial three times,” she told The News. “And now yet again!” 

The Gambinos aren’t the powerful crime family they used to be. The feds gutted the leadership in February when they arrested more than 60 mobsters in New York City and Sicily on charges that include murder, extortion and racketeering. Four other alleged New York wiseguys who had been in cahoots with the Gambinos were named in a related racketeering indictment. Also named was another childhood friend of Junior’s, 33-year-old James Cadicamo of Tampa, who is accused of conspiring to kill a Gambino thug who turned government witness. Another of the accused, 47-year-old John Burke of Queens, had been charged as the triggerman in the Gotterup case but was later acquitted. He was doing time for another crime in an upstate prison when he was hit Tuesday with the new racketeering charges. 

Guy Peden, 47, of Wantagh, L.I., was charged Tuesday with helping Burke kill Gotterup. Burke and David D’Arpino, 33, of Howard Beach, Queens, were also charged with murdering John Gebert in Queens on July 12, 1996. Gotti has not been connected to that crime. The other man indicted on racketeering charges Tuesday was Michael Finnerty, 43, of Oceanside, L.I. Like the others, he faces life in prison if convicted. 

Gotti was also charged with conspiring to distribute 5 kilos of cocaine, according to the indictment. He remains a suspect in the 1983 stabbing murder of Danny Silva during a brawl at the Silver Fox Bar in Ozone Park, but prosecutors decided the evidence was too weak despite the emergence of a new witness last year, sources said.
