Instructions

You will be completing a grant application as the lead organization to promote the widespread adoption, dissemination, and implementation of an important public health strategy (i.e., innovation). You may apply for up to $1 million to achieve your dissemination goals. The entire grant application should be no more than 20-pages double spaced in 12-point font, plus two tables and/or figures (optional).

TIPS FOR YOUR GRANT APPLICATION

  1. Be sure to write your application from the perspective of the lead org. Avoid using statements such as “I believe” or other language you may have used in previous assignments to express a personal opinion.
  2. You may use text from your previous assignments, although in most cases you will need to elaborate and/or further specify what you described in your initial assignments in order to develop a complete and cohesive picture of your innovation and dissemination plan. Make sure you address ALL feedback from previous assignments. 
  3. Most sections of the grant application should be written in narrative form except where the instructions indicate you should use the provided template. Avoid using bullet points to describe details. You may, however, use up to two tables and/or figures to summarize details in the format of your choosing.
  4. The goal of your application and dissemination plan is to describe how you will promote adoption and broadly disseminate the strategy to have the greatest reach within your allocated resources. Your plan should focus on a geographic area beyond a single city or community, e.g., within a state, region, or nation. 
  5. Your response should address all parts of questions/items within the instructions.

  1. Describe the public health problem or opportunity your organization is addressing. Include in your response the audience or population affected by the problem and who you will reach through broad dissemination of the strategy (i.e. beneficiaries). In order to do this, please specify the intended beneficiaries in terms of the populations, geographic regions, age groups, and other characteristics that clearly identify who you will reach through the dissemination plan. 
  • Describe the public health strategy you propose to widely disseminate or implement on a large scale. What evidence is there that this is the “best approach” to addressing the public health problem you have identified? Include your rationale for selecting this strategy and explain why it is ready for broad dissemination? Describe the “essential elements” of the strategy including the WHAT, HOW and WHO of the strategy  
  1. Dissemination Plan and Implementation (6-page limit) 
  1. Describe your plan for broadly disseminating the public health strategy. Discuss how you will use structures and processes that are already in place and what new structures you will need to create to reach the population/audience of interest and achieve broad dissemination. 
  • Explain why the selected public health strategy is well suited for broad dissemination within the selected populations/audiences/communities?
  • Describe any adaptations you will make to the prevention strategy to improve its fit for the selected populations/audiences/communities. Provide a strong rationale for how these adaptations will not compromise the essential elements of the strategy and why they are needed. 
  1. Implementing Organizations (3-page limit) 
  1. Describe the lead organization or group that will receive the funding and oversee the implementation of the selected public health strategy. Describe the lead organization’s strengths with respect to its capacity to broadly disseminate the strategy. Explain why this organization is ready to broadly disseminate the strategy. What capacities will the organization build to prepare for broad dissemination? 
  • Describe other organizations or groups that will participate in implementation. How will the lead organization prepare these organizations or groups to implement the strategy? 
  1. Evaluation Plan (3 pages including evaluation plan template)
