ASSESSMENT REQUIREMENTS
This is an assignment on a clinical decision-making report on the scenario written below. The Nurse looking after the patient who presented with this fluid overload below will write a report on the reason why she took the decision to get the patient a pressure-relieving mattress because of his risk of developing pressure ulcer upon doing a Waterlow assessment for the patient. Also, the decision to regularly reposition the patient, call the doctor to reduce the diuretic medication from 40mg to 20mg and change the timing of the medication from daytime to night-time. All these must be backed up with health policies like NMC, NICE guidelines, and other relevant literature.

THE CLINICAL SCENARIO
Anthony (anonymized) is an 89years older adult who presented with a broken hip following an unwitnessed fall at home. He was transferred from ED to the adult assessment unit. His medical history includes Heart failure, non-insulin-dependent diabetes, Hypovolemia, and chronic venous ulcer on the right leg. Upon admission to AAU, Anthony was immobile and incontinent of urine. He has been taking furosemide 40mg per day as prescribed by his physician. During his waterloo assessment under the supervision of my practice assessor, my practice assessor and I discovered that Anthony was at high risk of developing pressure ulcers due to his age, immobility, and urine incontinence. He consented to the treatment plan that involved getting a pressure-relieving mattress, frequent Repositioning, and regular pad changes to minimize the risk of developing pressure ulcers but contributed less to decision-making.
