Each appendix should have a tile and be denoted by a letter e.g. appendix A, B, C etc. and so on.

In each section, you should provide extracts which relate to that theme or discourse, and provide some analytic content. You should not just describe your extracts, but should tell the reader what is interesting about the data and why.
The best way to understand how a qualitative results section is formatted is to read journal articles which use your method of analysis.
